Eligibility Requirements

The following are required to participate in Middle School sports and can be completed in Skyward Family Access - Under Online Forms.

• Current year physical form/alternative year physical on file
• Concussion form
• Medical release of information
• Permission to play/handbook acknowledgement
• Sudden Cardiac Arrest form
• Fee Paid - $30 per sport (no fee for 6th-grade volleyball, basketball & tennis)
